I am writing to express my sincere and deepest apologies over the error I made with regards to your appointment with Mr. Steward yesterday.

I admit that I was in a hurry while taking his call and thus, wrote down the wrong date for your appointment. I was not too well and had rushed the call to visit the washroom. I am so sorry to have caused you much inconvenience and embarrassment over my error. I have written personally to apologize to Mr. Steward over my carelessness.

Please forgive me. I assure you that I will not repeat such an error. This is my first in the 7 years I have been working with you. I will be more careful from now onwards.
